The Designated Coordinator (DC) is responsible for overseeing the supervision, support, and evaluation of activities for individuals receiving services. The DC must ensure compliance with Coordinated Service and Support Plans, federal and state regulations, and organizational policies and procedures to ensure the quality delivery of services to all individuals served. The Designated Coordinator must be competent to perform the required duties through education, training, and work experience relevant to the primary disability of the individuals served.The Designated Coordinator’s responsibilities include:- Conducting periodic audits of individuals served case files to ensure accurate documentation of program-related services- Maintaining record-keeping systems and implementing correction plans as needed- Building and maintaining professional relationships with support team members and consultants- Making recommendations about home and community-based services and coordinating their delivery- Maintaining and summarizing data for reports on service outcomes- Developing and implementing programs to support organizational goals- Providing administrative oversight and ensuring compliance with regulations- Collaborating with team members to deliver comprehensive services- Managing and supervising a team of professionals- Overseeing program management activities- Building and maintaining relationships with stakeholders- Supporting the professional development of staff members- Staying up-to-date with industry trends and best practicesSkills required for this position include program development, administrative skills, experience in behavioral health or social work, team management, program management, relationship management, budgeting, and financial management.
