ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES1. Evaluates patients referred for home care.2. Collaborate with the hospital’s discharge planning personnel, utilization review department and insurance Case Managers to facilitate admission.3. Participates in patient care conferences and in-services.4. Collects and maintains statistical data on all referrals and submits them regularly as required.5. Attend meetings and participate on committees as required.6. Evaluates all elements of patient records daily.7. Maintains log of interim orders and plans of care to verify sent and return date to comply with state and federal regulations.8. Maintains adequate inventory of medical supplies/items needed to provide appropriate patient care, notifies the Clinical Supervisor when inventory needs to be restocked.9. Maintains adequate supply of patient clinical record elements and stock of pre-made clinical records available for clinicians.10. Answers telephone lines promptly and efficiently. Responds to message content appropriately, as instructed by the Clinical Director. Records telephone calls onto call log.11. Special projects and other related duties as assigned by the Clinical Director. Minimum of two (2) years’ experience, at least one (1) of which is in public health or home care nursing.2. Demonstrates an ability to work with other health care clinicians and knowledge of third party reimbursement.3. Self-directed and able to work with minimal supervision.4. Demonstrate excellent observation verbal and written communication skills.
