Hiring Social Worker – Client & Family Advocate at Woven Home Care, Inc., Kirkwood





Position OverviewThe Social Worker is the primary point of contact for clients and their families, ensuring they receive the support, advocacy, and resources needed for a high quality of life at home. This role allows Care Managers to focus on clinical assessments and care plan development while ensuring seamless communication and continuity of care. The Social Worker collaborates closely with clients, families, caregivers (Weavers), and healthcare providers to address psychosocial needs, coordinate services, and enhance overall well-being.Key Responsibilities:Client & Family SupportServe as the primary liaison between clients, families, and the Woven team, ensuring all needs and expectations are met.Conduct psychosocial assessments to identify client and family needs, strengths, and challenges.Develop and implement individualized support plans, addressing social, emotional, and practical aspects of care.Provide guidance on long-term care planning, community resources, and government benefits (e.g., Medicaid, VA, long-term care insurance).Offer crisis intervention and emotional support to clients and families, helping them navigate challenges such as caregiver stress, dementia progression, or end-of-life planning.Facilitate communication between clients, families, and the healthcare team to promote shared decision-making.Care Coordination & Quality AssuranceMonitor and review client information in Woven Connect, our proprietary client management system, ensuring accurate documentation of care.Oversee Weaver-recorded data from the Daily Checker, Pillbox, and Workflow Activities to ensure care plan adherence and identify any concerns.Track incidents, specific client notes, and critical updates to ensure timely follow-up and intervention.Monitor client satisfaction and address service concerns promptly to ensure the highest quality of care.Partner with Care Managers to adjust care plans based on changing client needs.Support clients transitioning between care settings (e.g., hospital discharge, rehab, or assisted living).Caregiver & Team CollaborationEducate and support caregivers regarding client needs, communication techniques, and best practices for delivering compassionate care.Participate in case discussions and care team meetings, offering insights on psychosocial aspects of care.Collaborate with the administrative team on service coordination, scheduling, and problem-solving.Community & Resource DevelopmentBuild and maintain relationships with community organizations, support groups, and healthcare providers to expand resource options for clients and families.Stay informed on local and national programs that may benefit clients and families.QualificationsBachelors or Masters degree in Social Work (BSW/MSW) required.Licensed Clinical Social Worker (LCSW) or LMSW preferred.Experience working with older adults, home care, hospice, or healthcare settings preferred.Strong interpersonal and communication skills with the ability to build trust and rapport with clients and families.Knowledge of community resources, benefits programs, and social service systems.Ability to manage multiple priorities while maintaining a compassionate and client-centered approach.Reliable transportation with valid drivers license and insurance.About Woven Home CareWoven Home Care is dedicated to making home the safest and most effective place to receive care.
